When you come in for your visit, any applicable copayments and deductibles will be collected at that time. After your insurance company processes the claim and we receive the Explanation of Benefits (EOB), we’ll send you a statement for any remaining balance. This balance will be charged to the card you have on file for your convenience and to avoid any delays in care.

If additional services such as labs, procedures, or medication administration are needed, they will be in addition to the visit fee. A $30 medication administration fee applies to all in-house injectables to support the safe and professional delivery of your care.

Before starting, an initial consultation with one of our providers is required to determine if you’re a good candidate for treatment. Following that, monthly check-ins with a provider are necessary to monitor your progress and make any needed adjustments to your dosage—ensuring safe, effective, and personalized care every step of the way.
Following your consultation with a provider, if lab work is required or requested, the associated rates will apply in addition to the visit fee. Should any abnormal results be identified, you will be personally contacted by a member of our staff or your provider with appropriate aftercare instructions and, if necessary, prescriptions or medication recommendations to support your treatment plan. All labs require either a $15 in-office venipuncture or Quest Diagnostics specimen collection fee.
